Would it be more effective to just put vitamin A directly on the skin? Or do you have to extract something? Seems like it would be easier than trying to get carrots to stay on the face.

Sure, it would be more effective and expeditious, but also more chemically processed. Creams that contain Retinol (Vitamin A) are knows as retinoids, you've probably heard of Retin-A. In any case, I am always for natural application (in this case, carrots) that has slow release of nutrients and is effective long term than application of chemically altered vitamins. If you do decide to topically apply Vitamin A, please consult a dermatologist.
